Officials and Coaches Officials Specific instructions as to how to fill out the CRB form - UPDATED July 2013. In Simple terms you need to Contact the UKA Welfare team at dbs@uka.org.uk to request a DBS application. Please include your full name and postal address and what role you have. Note that Beds County Polo Shirts, Fleeces and Waterproofs can be purchased by any official. They come embroidered with the Beds County Crest and your name if you wish.
